Draw 2 sets of blood cultures (peripheral + CVC lumens) and serum lactate immediately.
Clinical ActionAdminister Empiric Antipseudomonal Beta-Lactam within 60 Minutes
Use an appropriate empiric antipseudomonal beta-lactam (e.g., cefepime, piperacillin-tazobactam or meropenem) selected for local resistance risk and source. Give a full initial dose promptly, then adjust maintenance dosing for kidney function/RRT and optimize beta-lactam exposure (prolonged infusion/TDM where appropriate). Add MRSA-active therapy only when clinically indicated (e.g., shock with relevant risk, catheter/skin infection, pneumonia or known colonization).
Assessment StepEvaluate Specific Organ Failure & Host Complications
Respiratory Failure vs Tumor Lysis Syndrome vs CAR-T Toxicity vs Fungal Infection.
Differential Branches (3)
1Branch
Clinical ActionAcute Respiratory Failure: HRCT Chest within 24h + Early BAL (PJP / Aspergillus / DAH)
Trial HFNC/NIV with a low threshold for intubation when noninvasive support is failing (HIGH trial context). Treat suspected/confirmed PJP with TMP-SMX adjusted to host and renal function; adjunctive corticosteroids are evidence-based for moderate-to-severe HIV-associated PCP but should not be applied automatically to HIV-negative PJP. Use syndrome-appropriate mold therapy for suspected invasive aspergillosis with ID input.
2Branch
Clinical ActionTumor Lysis Syndrome: Vigorous IV Hydration + Rasburicase 0.2 mg/kg + CRRT
Rasburicase contraindicated in G6PD; CRRT for refractory hyperkalemia and hyperphosphatemia.
3Branch
Clinical ActionPost-CAR-T Toxicity: Tocilizumab 8 mg/kg for CRS vs Dexamethasone for ICANS
Grade with ASTCT criteria; avoid tocilizumab for isolated ICANS neurotoxicity.

An educational representation of clinical reasoning, not a diagnostic algorithm or personalized medical advice — always apply clinical judgment and institutional protocols.

Caution

Escalate — Red Flags & Critical Warnings

  • Rasburicase in G6PD deficiency triggers fatal methemoglobinemia and massive intravascular hemolysis.
  • In hyperleukocytosis/leukostasis, unnecessary RBC transfusion before cytoreduction can increase blood viscosity and worsen microvascular symptoms. Avoid reflex transfusion, but do not withhold lifesaving RBCs for major hemorrhage or critical symptomatic anemia; coordinate with hematology.
