Suspected Sepsis / Infection with Acute Organ Dysfunction
Assessment StepScreen for acute organ dysfunction: Delta SOFA >= 2Explore Topic
Evaluate PaO2/FiO2 ratio, platelets, bilirubin, MAP / vasopressor need, GCS score, and creatinine/urine output. Identify infected vs non-infectious systemic inflammation.
Clinical ActionApply SSC 2026 early treatment framework
Begin resuscitation immediately. Obtain cultures ideally before antimicrobials without harmful delay; give antibiotics ideally within 1 h for septic shock or probable/definite sepsis, and within 3 h after rapid evaluation for possible sepsis without shock. For sepsis-induced hypoperfusion/shock, give an individualized initial crystalloid strategy (SSC suggests at least 30 mL/kg in the first 3 h), start norepinephrine early when needed, and reassess perfusion dynamically.
Differential Branches (2)
1Branch
Decision PointIdentify anatomical infection source & pursue emergent source control (<6–12h)
Antibiotics alone will not sterilize closed-space infection. Urgent surgical/percutaneous source control is time-critical.
Clinical ActionDrainable Collection / Perforated Viscus: Emergent surgical/IR drainage
Laparotomy, percutaneous abscess drainage, nephrostomy for pyonephrosis, or infected line removal.
Clinical ActionNon-Drainable (Pneumonia / Cellulitis): Medical therapy & organ support
Optimize drug dosing (loading doses, continuous beta-lactam infusions), supportive ventilation, and hemodynamic targets.
2Branch
Reassessment / LoopReassess lactate clearance and perfusion indices at 3–6 hours
Monitor serial lactate downtrend interpreted in clinical context alongside capillary refill time (target <= 2s) and urine output (>= 0.5 mL/kg/hr); do not continue fluid boluses solely to normalize lactate once volume responsiveness is exhausted.
Reassessment / LoopAt 48–72 hours: Review microbiology & de-escalate antimicrobialsExplore Topic
Narrow broad-spectrum agents (vancomycin, anti-pseudomonals) based on definitive culture sensitivities; set total duration (typically 5–7 days for responsive uncomplicated infection).

An educational representation of clinical reasoning, not a diagnostic algorithm or personalized medical advice — always apply clinical judgment and institutional protocols.

Caution

Escalate — Red Flags & Critical Warnings

  • Every hour of delay in necessary surgical source control in septic shock significantly increases ICU mortality — involve surgical/interventional services immediately.
  • Continuing unguided broad-spectrum combination therapy beyond 72h without documented microbiological justification drives fungal superinfections and multi-drug resistance.
