ATS — ATS 2026 NIV for acute hypercapnic respiratory failure
ATS, 2026
NIV remains strongly recommended to reduce intubation and mortality in acute hypercapnic respiratory failure. HFNC is a conditional option only for selected less severe hypercapnia with mild acidemia, such as pH >7.25, where close monitoring and prompt escalation to NIV are available.

Disease ReferenceCore taxonomy (Types I–IV), alveolar gas exchange physiology, work-of-breathing mechanics, and escalation thresholds in acute respiratory failure.
Pathophysiologic mechanisms (shunt vs V/Q mismatch), dynamic ROX index trajectories, P-SILI prevention, and evidence-based oxygen escalation ladders.
Mechanism-based diagnosis of acute and acute-on-chronic hypercapnia, controlled oxygen, NIV, invasive ventilation, auto-PEEP troubleshooting, neuromuscular monitoring, and post-acute planning.
Pathophysiology of acute-on-chronic hypercapnia, controlled oxygen targets (88–92%), evidence-based NIV, and auto-PEEP matching mechanics.
