ATS — ATS 2026 support after extubation
ATS, 2026
After critical illness, consider HFNC or facemask NIV rather than standard oxygen. For high-risk extubation failure, conditionally favor NIV over HFNC or standard oxygen. Very-low-risk patients may need only low-flow oxygen or room air. Support must not delay necessary reintubation.

Disease ReferenceCore taxonomy (Types I–IV), alveolar gas exchange physiology, work-of-breathing mechanics, and escalation thresholds in acute respiratory failure.
Pathophysiologic mechanisms (shunt vs V/Q mismatch), dynamic ROX index trajectories, P-SILI prevention, and evidence-based oxygen escalation ladders.
Mechanism-based diagnosis of acute and acute-on-chronic hypercapnia, controlled oxygen, NIV, invasive ventilation, auto-PEEP troubleshooting, neuromuscular monitoring, and post-acute planning.
Pathophysiology of acute-on-chronic hypercapnia, controlled oxygen targets (88–92%), evidence-based NIV, and auto-PEEP matching mechanics.
