Individualised, short-course antibiotic treatment versus usual long-course treatment for ventilator-associated pneumonia
Lancet Respiratory Medicine, 2024 · Multicentre, randomized, open-label, hierarchical noninferiority-superiority pragmatic trial.
Clinical question
Can clinically individualized VAP antibiotic courses of 3–7 days be noninferior to usual courses of at least 8 days?
What the trial found
Selected clinically improving VAP patients can receive individualized courses shorter than traditional prolonged therapy without worse death/recurrence outcomes in this trial.
Current guideline position
Adds newer RCT evidence beyond the 2016 ATS/IDSA default 7-day recommendation; a new ATS/IDSA HAP/VAP guideline is in development as of Aug 2026.
Clinical interpretation
Supports stewardship and response-guided duration; it should not be interpreted as a mandate to stop every VAP course at day 3–5, especially with uncontrolled infection, abscess/empyema, bacteremia, profound immunosuppression or poor response.
Quantitative lower tract sampling (ETA vs BAL), rapid multiplex PCR panels and stewardship interpretation, PK/PD extended beta-lactams, 7-day duration, and the SHEA prevention bundle.
Nosocomial pneumonia developing ≥ 48 hours after admission in non-ventilated patients, host and antibiogram MDR risk tailoring, nasal MRSA PCR screening, and 7-day de-escalation.
