Extracorporeal CO2 elimination for acute exacerbation of severe COPD requiring invasive mechanical ventilation: a randomized controlled trial (the X-COPD trial).
Critical care (London, England), 2026 · Open-label randomized trial stopped for funding reasons
What the trial found
0/8 versus 3/9 evaluable patients; P=.21. A shorter ventilation duration was exploratory; one ECCO2R patient had severe bleeding.
Clinical interpretation
No established mortality/disability benefit. Do not adopt routine ECCO2R; liberation from the ventilator and from the extracorporeal circuit are separate decisions.
Limitations
Pathophysiology of acute-on-chronic hypercapnia, controlled oxygen targets (88–92%), evidence-based NIV, and auto-PEEP matching mechanics.
Mechanism-based diagnosis of acute and acute-on-chronic hypercapnia, controlled oxygen, NIV, invasive ventilation, auto-PEEP troubleshooting, neuromuscular monitoring, and post-acute planning.
Specialist-level VV/VA ECMO bedside management: selection, cannulation, gas exchange, hemodynamics, anticoagulation, circuit troubleshooting, organ support, ultrasound, ECPR, weaning and decannulation.
