High flow nasal cannula oxygen therapy versus non-invasive ventilation for acute exacerbations of chronic obstructive pulmonary disease with acute-moderate hypercapnic respiratory failure: a randomized controlled non-inferiority trial.
Crit Care, 2024 · Open-label noninferiority trial at two ICUs within one center
What the trial found
Failure 25.7% versus 14.3%; intubation 14.2% versus 5.4%. HFNC did not meet the noninferiority criterion.
Clinical interpretation
Supports NIV as default in acidotic AECOPD. ATS 2026 permits selected mild-acidemia HFNC use with close monitoring and prompt NIV escalation; the trial does not prove mortality superiority.
Limitations
Pathophysiology of acute-on-chronic hypercapnia, controlled oxygen targets (88–92%), evidence-based NIV, and auto-PEEP matching mechanics.
Mechanism-based diagnosis of acute and acute-on-chronic hypercapnia, controlled oxygen, NIV, invasive ventilation, auto-PEEP troubleshooting, neuromuscular monitoring, and post-acute planning.
