Cancer Patient with Abrupt Electrolyte Collapse

Hyperkalemia, hyperphosphatemia, hypocalcemia and AKI around cancer therapy.

Recognize

  • High tumor burden, rapidly proliferative or treatment-sensitive malignancy, high LDH, renal dysfunction and elevated baseline urate increase risk; spontaneous TLS can occur.

Act

  • High-risk patients need frequent laboratory reassessment and cardiac monitoring because hyperkalemia and hypocalcemia-related arrhythmias may evolve rapidly.

Reassess

  • Reassess trajectory, organ support, competing diagnoses and failure of first-line therapy; escalate early when physiology is worsening.
