Thyroid Storm — ICU Decision Pathway

Clinical diagnosis and ICU treatment of decompensated thyrotoxicosis with adrenergic, cardiovascular, neurologic and thermoregulatory failure.

Assessment StepThyroid storm is a clinical diagnosisExplore Topic
Severe thyrotoxicosis with fever, marked tachycardia/AF, heart failure, agitation/delirium, GI/hepatic dysfunction and a precipitant should prompt treatment; hormone levels do not reliably distinguish storm from uncomplicated thyrotoxicosis.
Clinical ActionControl adrenergic excess cautiously
Use a beta-blocker when appropriate, but titrate carefully in decompensated heart failure or shock; short-acting esmolol may be safer when hemodynamic tolerance is uncertain.
Reassessment / LoopReassess response and escalate
Trend physiology and organ function; if the patient is worsening, revisit diagnosis, source control and rescue options rather than repeating ineffective therapy.

An educational representation of clinical reasoning, not a diagnostic algorithm or personalized medical advice — always apply clinical judgment and institutional protocols.

Caution

Escalate — Red Flags & Critical Warnings

  • Escalate early for shock, rapidly progressive organ failure, dangerous ECG/airway findings or failure of initial therapy.
