Acute Neurologic Deterioration & Brain Death Workflow

From acute GCS drop and herniation emergency measures to emergent CT, neurocritical intervention, and death by neurologic criteria determination.

Acute Neurological Deterioration (Drop in GCS >= 2 / Pupillary Asymmetry)
Assessment StepStabilize airway & circulation; check fingerstick glucose immediatelyExplore Topic
If GCS <= 8 or loss of protective airway reflexes, perform rapid sequence intubation with neuroprotective precautions (avoid hypoxia and hypotension, maintain MAP >= 80–90).
Decision PointCheck for signs of acute brain herniation or severely elevated ICP
Signs: unilateral dilated fixed pupil, decerebrate/decorticate posturing, Cushing's triad (bradycardia, severe hypertension, irregular respirations).
Differential Branches (2)
1Branch
Clinical ActionHerniation Present: Emergent Osmotherapy & Temporary Hyperventilation
Administer 3% Hypertonic Saline (250 mL IV over 10–15 min) or Mannitol 20% (1 g/kg IV rapid bolus). Elevate head of bed 30 degrees. Hyperventilate temporarily to PaCO2 30–35 mmHg.
Assessment StepTransport immediately for Emergent Non-Contrast Head CT
Identify mass effect, midline shift, intracranial hemorrhage (ICH, SDH, EDH, SAH), hydrocephalus, or large hemispheric infarction.
Decision PointEvaluate for emergent neurosurgical vs medical neurocritical management
Surgical: emergent hematoma evacuation, decompressive hemicraniectomy, or EVD for hydrocephalus. Medical: targeted temperature management, ICP monitoring, antiepileptic therapy.
Assessment StepCatastrophic Unrecoverable Injury: Formal Brain Death EvaluationExplore Topic
When clinical criteria met: irreversible coma, absence of all brainstem reflexes (pupillary, corneal, oculocephalic, oculovestibular, gag, cough), and an apnea test consistent with BD/DNC: no respiratory effort, arterial pH < 7.30, and PaCO2 >= 60 mmHg AND >= 20 mmHg above the appropriate pre-apnea/chronic baseline, after excluding hypothermia, shock, and confounding CNS depressants. If chronic CO2 retention is suspected and the premorbid baseline is unknown, ancillary testing is additionally required after an otherwise qualifying apnea test.
2Branch
Assessment StepNo Herniation: Perform urgent Non-Contrast CT Head & metabolic panel
Evaluate stroke / intracranial hemorrhage, sepsis-associated encephalopathy, hepatic encephalopathy (ammonia), uremia, and drug intoxication.

Caution

Escalate — Red Flags & Critical Warnings

  • Even a single episode of hypotension (SBP < 90 mmHg or MAP < 65) or hypoxemia (SpO2 < 90%) in acute brain injury doubles patient mortality — aggressively defend cerebral perfusion pressure (CPP = MAP - ICP >= 60–70 mmHg).
  • BD/DNC determination must NOT proceed until CNS-depressant drug effects are excluded. Use measured drug levels when available or allow at least 5 drug half-lives, extending this interval for renal/hepatic dysfunction, obesity, hypothermia, prolonged infusions, or active metabolites. EEG does not demonstrate sedative clearance and is not an accepted ancillary test for BD/DNC under the 2023 consensus guideline.
