IDSA/ATS VAP 2016 — Management of Adults with Hospital-acquired and Ventilator-associated Pneumonia
IDSA / American Thoracic Society, 2016
A 7-day course is the 2016 default for most improving HAP/VAP patients, with shorter or longer treatment individualized to clinical response and infection complications. Empiric therapy should be guided by local antibiograms and patient-specific MRSA/Pseudomonas risk. As of 27 Aug 2026 this remains the published ATS/IDSA treatment guideline; IDSA lists a replacement HAP/VAP guideline as in development for Q4 2026, not yet final.
Clinical Context & Relevant Topics (3)
Disease ReferenceQuantitative lower tract sampling (ETA vs BAL), rapid multiplex PCR panels and stewardship interpretation, PK/PD extended beta-lactams, 7-day duration, and the SHEA prevention bundle.
Nosocomial pneumonia developing ≥ 48 hours after admission in non-ventilated patients, host and antibiogram MDR risk tailoring, nasal MRSA PCR screening, and 7-day de-escalation.
Pattern-based ICU pneumonia approach: severe CAP, HAP/VAP, aspiration, viral, necrotizing, immunocompromised and nonresolving pneumonia.
