ACC/AHA 2020 — 2020 ACC/AHA Guideline for the Management of Patients With Valvular Heart Disease
ACC / AHA, 2020
In patients with symptomatic severe aortic stenosis, intervention (SAVR or TAVR) is recommended to improve survival and quality of life (Class 1, LOE A). In acute severe MR due to mechanical causes or acute severe AR with hemodynamic compromise, urgent surgical intervention is indicated (Class 1, LOE B-NR).
Clinical Context & Relevant Topics (3)
Disease ReferenceCritical AS, acute AR/MR mechanics, prosthetic valve emergencies, and infective endocarditis with multidisciplinary surgical triggers.
Primary pump failure, SCAI staging (A–E), PAC hemodynamics (CPO, PAPi), inotrope selection, emergent revascularization, and mechanical circulatory support.
Hemodynamic phenotypes (SCAPE vs ADHF vs Cardiogenic Shock), non-invasive positive pressure ventilation, high-dose vasodilator algorithms, loop diuretics, and sequential nephron blockade.
