For Forrest IIb adherent clots, conditionally consider removal and treatment when the endoscopist can safely manage bleeding. OTS clips are an alternative for selected high-risk ulcers and recurrent bleeding. Selected very-high-risk patients may be considered for prophylactic embolization. Resume necessary anticoagulation when clinically indicated by thromboembolic risk; durable hemostasis permits oral nutrition within 24 hours.
