For hemodynamically stable hospitalized adults without a hemoglobinopathy, cyanotic heart disease, or severe hypoxemia, use a restrictive transfusion strategy, transfusing at a hemoglobin threshold below 7 g/dL (strong recommendation, moderate certainty). Consider a threshold of 7.5 g/dL for cardiac surgery and 8 g/dL for orthopedic surgery or pre-existing cardiovascular disease.
