Acute RV failure resuscitation requires careful volume optimization (avoiding overfilling that exacerbates ventricular interdependence), restoration of systemic blood pressure (norepinephrine or vasopressin to maintain RV coronary perfusion gradient MAP - CVP), inotropic support (dobutamine or milrinone), and reduction of RV afterload using inhaled pulmonary vasodilators (inhaled nitric oxide or inhaled epoprostenol).
