2026 US PE Guideline — Guideline for the Evaluation and Management of Acute Pulmonary Embolism in Adults
AHA/ACC/ACCP/ACEP/CHEST/SCAI/SHM/SIR/SVM/SVN, 2026
Use the 2026 AHA/ACC acute-PE clinical categories to match monitoring and reperfusion intensity to severity. Advanced therapies are reasonable in Category E1 and can be considered in selected D1-D2 patients; systemic thrombolysis is also reasonable in E2 when appropriate to the arrest/refractory-shock context. Choice among systemic thrombolysis, catheter-based therapies, surgical embolectomy, and mechanical circulatory support depends on bleeding risk, anatomy, shock/arrest phenotype, time to treatment, and local expertise. PERTs are recommended to improve timely individualized decision-making.
