ESC HF 2026 — 2026 ESC Guidelines for the management of heart failure
European Society of Cardiology, 2026
Updates heart-failure classification, prevention, diagnosis and therapy. The ESC framework now uses two LVEF phenotypes—HFrEF with LVEF <50% and HFpEF with LVEF >=50%—and adds stage-based terminology emphasizing prevention, early recognition and timely intervention. Acute/decompensated presentations remain phenotype- and hemodynamic-context dependent rather than governed by one universal treatment recipe.
Clinical Context & Relevant Topics (3)
Disease ReferenceHemodynamic phenotypes (SCAPE vs ADHF vs Cardiogenic Shock), non-invasive positive pressure ventilation, high-dose vasodilator algorithms, loop diuretics, and sequential nephron blockade.
Hydrostatic pulmonary edema physiology, SCAPE vs decompensated HF vs cardiogenic shock, NIV, titrated vasodilators when hypertensive, diuresis, and sequential nephron blockade.
Primary pump failure, SCAI staging (A–E), PAC hemodynamics (CPO, PAPi), inotrope selection, emergent revascularization, and mechanical circulatory support.
