ELSO VA-ECMO 2021 — ELSO Interim Guidelines for Venoarterial Extracorporeal Membrane Oxygenation in Adult Cardiac Patients
Extracorporeal Life Support Organization, 2021
Adult VA-ECMO should be used as a time-limited bridge in carefully selected patients with potentially reversible or bridgeable cardiac failure. Management requires attention to cannulation strategy, adequate systemic perfusion, LV loading and aortic-valve opening, differential hypoxemia, limb perfusion, anticoagulation, complications, and repeated assessment for recovery, durable support or transplantation.
Clinical Context & Relevant Topics (2)
Disease ReferenceSpecialist-level VV/VA ECMO bedside management: selection, cannulation, gas exchange, hemodynamics, anticoagulation, circuit troubleshooting, organ support, ultrasound, ECPR, weaning and decannulation.
Primary pump failure, SCAI staging (A–E), PAC hemodynamics (CPO, PAPi), inotrope selection, emergent revascularization, and mechanical circulatory support.
