AHA 2017 — Management of Cardiogenic Shock: A Scientific Statement from the AHA
American Heart Association, 2017
Emergency revascularization (PCI or CABG) is indicated for cardiogenic shock complicating acute MI. Norepinephrine is the initial vasopressor of choice to maintain coronary perfusion; inotropes (dobutamine or milrinone) are added for low cardiac index; routine IABP is not recommended.
Clinical Context & Relevant Topics (3)
Disease ReferencePrimary pump failure, SCAI staging (A–E), PAC hemodynamics (CPO, PAPi), inotrope selection, emergent revascularization, and mechanical circulatory support.
Plaque rupture, coronary thrombosis, STEMI vs NSTEMI risk stratification, emergent reperfusion, DAPT/anticoagulation, and mechanical/electrical ICU complications.
Hemodynamic phenotypes (SCAPE vs ADHF vs Cardiogenic Shock), non-invasive positive pressure ventilation, high-dose vasodilator algorithms, loop diuretics, and sequential nephron blockade.
