AHA/ERC/ILCOR Post-Arrest 2021 — International Consensus on Post-Resuscitation Care and Neuroprognostication

AHA / ERC / ILCOR, 2021

Post-ROSC care requires targeted temperature management: actively prevent fever (temperature <= 37.5°C) or maintain targeted hypothermia (32-36°C) for at least 72 hours in comatose patients. Maintain MAP >= 65 mmHg, target normoxia (SpO2 92-98%) avoiding hyperoxia, and target normocapnia (PaCO2 35-45 mmHg). Immediate coronary angiography is indicated for post-arrest STEMI or cardiogenic shock. Multimodal neuroprognostication must be delayed until >= 72 hours post-ROSC (incorporating clinical exam, SSEPs, EEG, serum NSE, and brain MRI/CT).
